The cause of death, as recorded on the death certificate, was listed as

1a Myocardial infarction
1b Atherosclerosis of coronary arteries
2 Carbon monoxide poisoning

Myocardial infarction is usually the result of coronary artery occlusion.

In this case, it developed because of a combination of impaired bloodflow (due to atheroma) and impaired oxygen carriage due to carbon monoxide poisoning.
